Gift Aid Declaration

If you are donating or have donated directly to Raise, and you are a UK taxpayer, please complete this form to allow us to claim Gift Aid. Gift Aid is effectively a rebate of income tax you paid as you earnt the money yo donate, and enhances the value of your donation by 25%. This means that if you donate, for example, a tenner, the charity will in fact receive £12.50

Complete this form and Raise (West Herts Hospitals Charity) will be able to claim an extra 25p for every £1 you give – at no extra cost to you.
